Employees must wash hands before donning gloves. COS - manager retrained staff. Employees cutting lemons & tomatoes with bare hands. COS - hands washed & gloves donned. Utensils stacked wet & cheese buckets on floor. COS - Utensils rewashed & stored 6" off floor, allowed to air dry. Warewashing machine 86F, must maintain a minimum of 120. COS - Manager made service call. 3 compartment sink to be used until repaired. Clean equipment interior of unused ice machine has debris.

Sausage, beef, & chile are uncovered in WI cooler. Container of flour cannot close in dry storage. Pan of cut lemons uncovered. COS - foods covered, new container to be bought for flour, per manager. Ware washing machine needs to dispense chlorine sanitizer at a minimum of 50 ppm. Use 3 compartment sink or manually add every cycle until repaired. Accumulationof slime in ice machine & debris on can opener. Items cleaned & sanitized. Clean accumulation of grease around vent on roof & in rain gutters. Clean more frequently.

Ensure utensils completely air dry before stacking. Invert utensils to prevent standing water on surface. Transfer canned food to food grade container once opened. Wash/Rinse temp on ware washing machine must reach a minimum of 120F. Grease along vent of roof & in rain gutters.Much grease has been removed, continue to clean & repair to prevent future grease.

Bulk onions badly molded & mixed with "healthly" looking onions. Monitor foods for adulteration. COS - all onions discarded. Foods must be maintained at 41F or less. COS food freshly prepared placed with more ice. Foods previous day discarded. Grease accumulated on roof. Running into gutters & downspouts. Remove grease immediately & clean hoods frequently enough to prevent from accumulating outside of building.
