No sanitizer registering on test strips, grease/film in solution. COS - fresh solution prepared. Allow utensils to air dry before stacking. Have wash temperature on warewashing machine repaired. Should reach 160F or manufactureres recommendation. Remove tapes/labels from containers when cleaning.

Food residue on tea urn nozzle & stored utensils. COS - Manager washed, rinsed & sanitized urn spout & other items taken to be cleaned. No sanitizer registering on test strips for sanitizer buckets, cloths out on counter tops. COS - fresh buckets prepared to 100 ppm chlorine & cloths stored submerged. Repair leaking plumbing at handsink to prevent water from spilling onto floor.

Recommend splash guards. COS - Rack moved.Sanitizer in buckets not reaching minimum required concentrations. Cloths out on counter & table tops. Keep submerged in sanitizer between uses. COS - Fresh buckets prepared.

Need a PIC on all shifts who can demonstrate knowledge of basic operational steps performed in the facility. Retrain PIC's in areas of logging time as a control for potentially hazardous foods & understanding the different types of sanitizers used in the facility. PIC should understand designated sanitizers require designated test strips. Food should not be consumed in food prep area. Food residue on stored utensils. Change sanitizer sink frequently enough to maintain required minimum sanitizer concentration. COS - utensils rewashed. Blocks of cheese at room temperature with no time log. Employees unsure when food removed from cold holding. Keep record keeping current when using time as a control for potentially hazardous foods. COS - cheese discarded. Need quat test strips if using quat sanitizer to sanitize utensils.

Post hand washing signage at hand sinks. Utensils stacked wet nested, allow to completely air dry before stacking. Keep utensils at least 6" off floor. Repair broken, loose, & missing cove base tiles, beside tea urn machine & behind bread racks. Dead insects under shelving in dry storage & behind tanks. Currently receiving pest control treatments.
