Bag onions on floor. COS - moved at least 6" off floor by manager.Employees assembling foods need a hair restraint. Store utensils at least 6" off floor.(dry buckets). Repair floor in WI cooler & hole in wall beside front handsink. Clean accumulated debris along floor/wall juncture throughout kitchen, wipe down walls routinely.

Employee cutting/handling lemon wedges with bare hands. Maintain no bare hand contact with ready to eat foods. COS - hand washed, gloves donned. Half eaten sandwich & open cups in food prep area. Avoid eating & keep drinks in single service cups w/ tight fitting lids & straws. COS- Food discarded & approved cups used. No soap at front kitchen hand sink. COS - employee restocked. Ware washing machine not dispensing sanitizer. COS - maintenance called & 3 comp sink used until repaired. Employees handling/assembling foods need a hair restraint.

No paper towels at employee bathroom, no soap at customer bathroom, keep properly supplied. COS - Soap & paper towels stocked. Water leak being caught in bucket in food prep area. Repair leak to prevent potential contamination from splash. Keep boxes of food at least 6" off floor in WI freezer. Employee handling & preparing foods must have a hair restraint. Keep wiping cloths stored submerged in buckets between uses. Inspection posted is not most recent. Post most recent inspection for public view. Water at front handsink being cut off below sink due to leak. Repair so hot & cold water is available at handles of sink to promote proper handwashing practices. Back door at loading dock being left wide open, need to install a self closer to help prevent fly entry.

Must cook/prepare foods indoors or in an enclosed outdoor area approved by the health department. COS - Foods moved. Employees preparing and/or handling foods must wear a hair restraint. No sanitizer in wiping buckets. COS - Fresh buckets prepared at 100 ppm chlorine. No hot & cold water available at front handsink handles. Cold water must be turned on below sink. Repair so that hot & cold water is available at handles. Repairing sink will allow cooks to wash hands before donning gloves much easier.

Foods hot held must maintain 135F.COS-Reheated on eye of stove to 165F & placed in new hot pot that can maintain foods @ 135F or above. Repair borken faucet at front handsink&leaking faucets at veggie sink. Need hot & cold water available at the faucet with no leaks.Accumulation of debris at floor/wall juncture in prep areas. Clean frequently & thoroughly especially under sinks & shelving.
